This two-tiered, humanitarian approach to healthcare that rewards Iowa’s workforce while guaranteeing essential care to all.
This page describes Tier 1, which all Iowans will have access to.
Tier 2, which (1) greatly rewards Iowa’s workforce, (2) Stimulates the healthcare industry’s economy by allowing insurers to cover electives and subsidizes them to help expand their networks, offering higher paid contracts to surgeons and medical staff, is described here.
Tier 1 — How it Works

Every Iowan is automatically enrolled in baseline medical coverage that includes emergency services, hospitalization, preventive care, prescription drugs, maternal health, and mental health services.

Funded by a graduated healthcare contribution, scaled by income:

| Under $35,000 | 0% |
| $35K–$80K | 1% |
| $80K–$150K | 2% |
| $150K–$500K | 3% |
| Over $500K | 5% (no opt-out) |

Those earning under $500,000 can opt out if they prefer private coverage — but most won’t want to. The care is excellent.

Iowans working in the healthcare field (including EMTs, nurses, CNAs, and hospital support staff) are exempt from the tax and receive full coverage, in recognition of their service.
